Complete our Notice of Potential Claim, Claim, Suit, Disciplinary Complaint Or Action Complaint, or Subpoena, print it out, and submit it along with all correspondence and relevant documents to claims@isbamutual.com, fax it to (312) 379-2049, or mail it to:

Although your telephone call is important, you must provide us with written notice of an ARDC complaint or other disciplinary action. E-mail, fax, or mail your completed Claim or Incident Report (which we use for reporting ARDC complaints and other disciplinary actions) or provide the following information:
• A copy of the letter notifying you of the complaint or disciplinary action
• The damage or injury that has or may result
• The identity of the claimants
• The date when you first became aware of the underlying issue
• A summary of the circumstance giving rise to the complaint or disciplinary action
